Just print out copies of this order form and have your customers/contacts fill out their order information  You take the order forms and process them on our website using your affilaite weblink or code.  We process the order and it gets shipped directly to your customer!  

Please just remember to let them know that you will be processing their order, but that the charge will be coming from Fitness Anywhere.  Also, shipping fees may apply and if they live in California, sales tax will be applied as well.

This is awesome...thanks.  I've given my business card to people with my affiliate link written on the back, but you never know if they are using it or not. 

This order form is a great! I've been asked so many times if someone could buy a TRX from me and I've had to hope and pray they use my code. I like the control this gives us affiliates, thanks for enhancing our ability to kill it!

Also, for camps and large events, is it possible to purchase bulk quantities at a discounted price for on-site sales? Thanks in advance for any feedback on this.

 Almost forgot, I was curious if the customer will receive a $20 Thank You from Fitness Anywhere for their purchase...in the form of a certificate that comes with their product or?...

This would certainly help the cause. :)
